Attention: The barcode in the top left of the printed form must include block letters of AUS and 9 digits (for example AUS170012345). In case, the barcode is ---- 170012345, the applicant must fill another online form to get a new barcode.
Original passport with at least 01-year validity if applying for a visa exemption Certificate. In special case, you can send a copy of your passport's front page to get a detached visa exemption certificate (in the form, you must choose "a detached certificate"). However, "a detached certificate" is only issued in very limited circumstances. The issuance of "detached certificate" is temporary stopped.
02 passport-size photos.
For person of Vietnamese origin: please enclose with the certified copy of Vietnamese documents like Vietnamese passport, Vietnamese birth certificate, Vietnamese Identity Card, etc.
For other applicants, please enclose with the certified copy of: (1) Marriage Certificate and Vietnamese documents of his/her spouse; or (2) Foreign Birth Certificate and Vietnamese documents of his/her parent.
A return prepaid self-addressed registered/express envelop.
Fee (in AUD by Money Order, or Bank Cheque made out to "The Embassy of Viet Nam").

Attention:
For foreigners who are wife/husband of Vietnamese origin, please send certified copies of (no more than 6 months): marriage certificate, paperwork that prove the Vietnamese origin of her husband/his wife (Vietnamese birth certificate, Vietnamese ID, Vietnamese passport...).
